Need Health Insurance

Central Montana Community Health Center provides access to dental services through the use of contracted dentists within the community.

CMCHC determines patient’s eligibility to utilize dental services during the intake process. When a patient requests dental services they will need to complete the intake form and provide proof of identification, current demographic information and insurance. Patients who have dental insurance will be instructed to follow up directly with a dentist of their choosing.

Patients who do not have insurance are required to provide proof of income and apply for the sliding fee discount. The sliding fee scale will determine which/if the patient qualifies for the Dental Program.

Patients will be scheduled for an appointment with CMCHC provider after income eligibility is verified by the Billing Department.

Patients eligible for dental services will sign an acknowledgement that dental limit per person per fiscal year (beginning May 1st – April 30th) is $250.00. All consumers that qualify for the sliding fee scale are eligible for this dental service. Dental referrals will not be processed until dental copays are paid in full to CMCHC.

This health center is a Health Center Program grantee under 42 U.S.C 254b of the United States codes,
and its providers and staff are deemed to be Public Health Service employees under 42 U.S.C 233 (g)-(n).
